But the biggest sin of “Smile” is that it just isn't scary. The movie has to rely on fakeout jump scares like a burglar alarm and even opening a can of cat food because it knows it can't pull off scenes with actual danger or violence. A bad dream sequence can't be taken seriously because Rose would never act that way.
7/10, this is the rating I'd give if I hadn't watched a lot of horrors so that's what I'm sticking with, it reminds me so much of The Ring and has a lot of the orthodox methods other horror movies used to incite fear. So nothing groundbreaking, still, for the most part they are stilly effective.
Every 15 minutes there was another graphic suicide scene, and/or some scene involving a psychotic break. In my opinion, they could've had the same storyline without the gore. Rose's body is smiling while she burns away, and we can see in Joel's eyes he will be scarred for life by that disturbing suicide. Joel's life won't last long, though, as he's now the entity's next victim. In the end, there's no way to escape trauma.
Smile does fit neatly in the psychological, jump–scare horror genre, reminiscent of It Follows. However, the film contains intense themes surrounding mental illness and the stigma surrounding it, which comes off as highly offensive rather than informative.
Smile's messy treatment of mental illness and trauma throughout one hour fifty-five minutes seems to be intentional and is bound to leave you feeling uncomfortable. The theme of inheriting mental illness and walking down a path that you have lived dreading all your life is poignant.
Smile turns that into a literal monster only the victim can see, as though it's a hallucination. The parallels to actual psychosis are striking, and that's why everyone around the main character Rose Cotter initially believes it to be a post-traumatic breakdown as a result of witnessing a suicide.
Why Rose Dies In Smile's Ending. Rose dies in Smile's ending because she was only able to overpower the entity once. She had enough fight in her to take on the gangly demonic monstrosity and even appeared to kill it, shattering a lantern over its head.
In the end, Rose goes to her childhood home to confront her unresolved trauma regarding her mother's death, in hopes of defeating the monster. There, she fights a giant, deformed version of her mom and sets the house on fire, before driving to her ex Joel's house (Kyle Gallner) for safety.
We learn that the young Rose allowed her mother to die rather than call for help because she was both afraid and angry with her mother. The demon believes this long-repressed revelation will paralyze Rose with guilt, making the task of subduing and overtaking her easier.
Trauma is very prevalent in this film. Rose, from the very beginning, is known to have experienced trauma as a child, seeing her mother overdose in front of her. That trauma is laced with guilt as she could have called for help in her mother's dying moments — but chose not to.
The smile curse operates in a fairly consistent fashion throughout the film. One infected person is consumed, the entity forces them to take their own life and then the witness becomes cursed. But, what if multiple people witness that death? Could more than one witness carry on the smile curse?
This film is far from as graphic and disturbing as Ari Aster's “Hereditary” and “Midsommar,” but there are still some disturbing scenes that make members of the audience want to plug their ears and squeeze their eyes shut. The jump scares are also intense but there aren't so many as to overdo it.

We had read this study that was from back in the sixties where they found out that this sort of Pepto Bismol pink color somehow was supposed to make violent inmates less violent.
