***Spoilers Below*** Anyone who's done bookkeeping knows that “red in the ledger” means debts that are unpaid. We've never learned the specific wrongs that haunt Black Widow, but we do know from the original Avengers that she credits Hawkeye with saving her from that life.
All the Widows who passed through the Red Room conditioning went through hysterectomy. Their wombs, ovaries and fallopian tubes were removed to make them sterile and unhindered by social involvements like having children and raising a family. Natasha Romanoff could never be pregnant.
According to Arnim Zola, Natasha Romanoff was born in 1984. Sometime shortly after that, she was indoctrinated by the Red Room, a Russian training program that took young girls and turned them into lethal undercover agents.

Despite a lifetime of conditioning, Natasha chose to pursue her own destiny and defected. The KGB was not eager to let their operative go and first extorted and then brainwashed her into submission, forcing her to fight the Avengers (which Hawkeye had joined in the meantime).
The Red Room is featured in the 2021 film Black Widow, in which the leader of the organization is named Dreykov, portrayed by Ray Winstone. The organization trained millions of "Black Widows" assassins until Natasha Romanoff and Clint Barton bombed Dreykov's offices, supposedly ending the program.
Traditionally, the Red Room operatives were trained from childhood to be spies and assassins. Well first there's the allure of the beautiful woman -- they have the option to use their charms to gain access and influence by becoming the lovers and companions of their targets.
Why does Yelena have a Russian accent but Natasha doesn't?
There are two possible explanations - either the Red Room training was so thorough her accent was embedded naturally rather than taught, or Natasha decided to carrying on using her U.S. voice as a way of escaping the emotional trauma of her previous life.

Natasha chose to be the sacrifice and so, Romanoff was 39 years old when she died, but the time travel in Avengers: Endgame creates a little bit of a complication.

Why does Natasha and Yelena have different last names?
Yelena's full name follows an incorrect style of naming in Russian, which is quite an odd error by Marvel. Her lack of a patronymic name helps to prove that she and Natasha aren't biologically related but actually are adoptive sisters, which is shown in the MCU.
The Russo brothers, who directed Infinity War and Endgame, once said that Wanda intentionally dropped the accent because she was training to be a spy and the accent would give her away.
After a fight with her cousin, John Reed, Jane is imprisoned in the red-room as a punishment. Jane is terrified of being left in the red-room because it is supposedly haunted by the ghost of her dead uncle.
She, like other Black Widow assassins created by the dreaded Red Room, was subjected to a forced hysterectomy during training, and therefore cannot menstruate. But Black Widow doesn't let Yelena become the butt of a joke.
Niko Constantin was the only male trainee of the Red Room's male equivalent dubbed "Wolf Spider", Niko Constantin proved to be an effective killer, but impossible to handle or control, leading to the program to be declared a failure.
Bucky ended up at the Red Room, an assassin training facility, where he participated in the Black Widow Ops program as a trainer—and that's where he met Romanoff for the first time.
She makes the sacrifice for the family she wants to be with. Thanks to Hawkeye we know that Natasha didn't have Yelena. Melina and Alexei we can't be sure, but Yelena was one of those snapped away. Meaning Natasha also sacrificed herself for her sister.
