The movie focuses on mental health and trauma and it does that very well. Not for the easily rattled, lots of gore and jump-scares. I would not recommend for kids.
Smile is rated R by the MPAA for strong violent content and grisly images, and language. Violence: People are seen cutting, stabbing, and burning themselves and others. A dead cat is shown.
The main antagonist of the film, a parasitic demon that attaches to each person for 7 days (making them kill themselves while smiling), is on the loose and eventually does appear in the film in its own jumpscare. The menacing entity attacks Rose towards the end of the film in its own eerie way.
However, in a twist that is as agonizing and frightening as it is brilliant, we see that all this was just another hallucination induced by the monster. The most terrifying moment comes when you realize that she never beat it or escaped, and is still trapped in the house.

Does smile have a lot of jump scares?
Few movies have used it effectively, where you're genuinely scared about what will happen. Andy Muschietti's It is filled with jumpscares, yet it's one of the most terrifying films of the last decade. Why? Because it knows how to subvert audience expectations and use jumpscares through moments you can't predict.
This film is far from as graphic and disturbing as Ari Aster's “Hereditary” and “Midsommar,” but there are still some disturbing scenes that make members of the audience want to plug their ears and squeeze their eyes shut. The jump scares are also intense but there aren't so many as to overdo it.
The film sustains a strong sense of foreboding and threat throughout, with central themes of trauma and death. There are frequent jump scares and some very strong violence. This movie is incredibly scary and disturbing at the same time.
For most of the movie, Rose is left to face this demon on her own, but it's clear she's been forced to deal with her demons all by herself for years. The stigma that can come with mental illness has taught Rose to smile through her grief so as not to make others uncomfortable.
Smile is a first person horror game that utilizes time management and fear of the unknown, along with semi-difficult gameplay, in order to create a truly haunting experience. A deeper story that is open to interpretation leaves the player wondering why we are truly here and if there is really any way to escape.
11-13 year olds may be better equipped to navigate the scary movie scene on some level but each child is different. If your child watches something that they say is not scary to them but then starts coming to you at night wanting to sleep closer to you or is having nightmares let their behavior speak for them.

Great film, but not for kids under 13. I recommend this film to everyone who loves slightly disturbing films and TV shows like me but for anyone under the age of 13 you really should not watch it. It's very gory and contains adult language.

Take a deep breath and relax the muscles in your face before you start smiling. It might help to close your eyes quickly and then open them back up. Think about something funny. If a natural smile just isn't coming, try to make yourself chuckle.
But the biggest sin of “Smile” is that it just isn't scary. The movie has to rely on fakeout jump scares like a burglar alarm and even opening a can of cat food because it knows it can't pull off scenes with actual danger or violence. A bad dream sequence can't be taken seriously because Rose would never act that way.
