On 1 March, 1997, Ron Weasley was poisoned when he drank some poisoned oak-matured mead that was intended for Albus Dumbledore. Harry, remembering the above mentioned incident in Potions class, quickly found the bezoar he had given Professor Slughorn and shoved it down Ron's throat, saving his life.
Why did Slughorn want to poison Dumbledore? He didn't. Draco did wanted to poison him because he was tasked with killing him. He put the poison in what was supposed to be a gift to dumbledore unbeknownst to slughorn.
How did Ron get poisoned in The Half-Blood Prince?
Slughorn pulls out a bottle of oak-matured mead he intended to give Dumbledore for Christmas and pours Ron a glass. Ron drinks his before the toast is over and suddenly shoots up in the air, his eyes bulging and foam coming from his mouth.
Afterward, Slughorn felt that Ron needed a pick-me-up. He gave Ron a glass of oak-matured mead, from a bottle that he had originally intended as a Christmas present for Professor Dumbledore. Unbeknownst to him, the mead had been poisoned by an Imperiused Madam Rosmerta.

Why is Snape the Half-Blood Prince?
Family. Snape's family background is mostly shown in flashbacks during the course of the last three novels. Snape was born to Eileen Prince, a witch, and Tobias Snape, a Muggle, making him a half-blood (hence the name, "Half-Blood Prince").
Throughout the 1996–1997 school year, Draco Malfoy would go to the washroom to find company with Moaning Myrtle, confiding the stress of his mission and fear of failing it.

What did Ron remember from the night he was poisoned?
Ron realized he was about to spill the beans and cut himself off; he wasn't ready to reveal his true feelings and basically chickened out. Later, when the trio was taken to Malfoy Manor, Bellatrix Lestrange isolated Hermione for interrogation by torture. Ron begged in vain to be taken in her place, and after being thrown into the cellar, Hermione's screams from above caused Ron to cry out her name in agony, eventually bringing him to tears.
What happened between Ron and Hermione after he was poisoned?
Hermione, furious, attacks Ron with a flock of magical canaries, and the two stop speaking. In the end, though, when Ron is poisoned, it is Hermione that he asks for as soon as he wakes up, and his relationship with Lavender comes to an end, leaving Ron and Hermione still not official, but clearly interested.

His tears were due to the fact that since the bird had died, he had failed once more to repair the Vanishing Cabinets. Draco needed to repair them in order to give the Death Eaters a safe way to infiltrate Hogwarts. Failure meant death, Voldemort had made that very clear.
It's implied that Septum Sempra is Snape's own creation, that he used a specific counter-curse to heal Draco, and that the spell isn't designed to be instantly fatal, but to inflict a gruesome, but relatively slow death if left untreated. It's likely Snape is the only person who knows said counter-curse.
