Chucky made his first appearance in the 1988 film Child's Play. In the film, a serial killer named Charles Lee Ray, aka Chucky (Brad Dourif) uses a voodoo ritual inside a toy store to transfer his soul into a Good Guy doll to escape from Detective Mike Norris (Chris Sarandon).
Charles Lee Ray was a character known as the Lakeshore Strangler who murdered 22 people before his death in the beginning of Child's Play. He is Chucky's original human form before he transferred his soul into a doll.
However, In both Seed of Chucky and Syfy's Chucky, Lee Ray claims he doesn't want to keep trying to transfer his soul into a human anymore and instead wants to stay a doll forever. This is because he has proved time and time again that he is far more dangerous as a killer doll.
Also known as the Lakeshore Strangler, we first meet Chucky in his original human form as he's running from the cops in Chicago. Upon being shot, he uses voodoo and an amulet, the Heart of Damballa, to transfer his soul into the closest vessel: a Good Guy doll.

Why did Chucky turn evil?
He only became evil due to his sadistic desire to hurt and kill others, as shown when his father was killed by the Hackensack killer, and decided to become just like him because he was inspired by the killers actions.
But the first movie showed it as something more parasitic since Andy could never escape the pint-sized tormentor. At first, Chucky wanted to use Andy's body as a vessel for his soul, but the more Andy fought back, the more he became another name on Chucky's hit list.
Despite being a doll, he has spent so long in his doll body that he has turned anatomically human. Chucky can also bleed and feel pain just like a normal human and can be easily overpowered, due to be around the same height as a young child.

Our own history is littered with killers on par with the fictional Charles Lee Ray, whose name is actually a mashup of three real-life criminals: “Charles” is a nod to Charles Manson, “Lee” to Lee Harvey Oswald, and “Ray” to James Earl Ray.

Throughout the first three films he looked like an everyday child's doll, but after his death in Child's Play 3, Chucky had his body stitched up to create his more popular look.
